Care Coordinator RN - Part Time / Weekends

About the Role

The Care Coordinator is an experienced clinical professional responsible for guiding and educating patients and families to manage their health effectively. They coordinate care for a designated patient population, considering medical needs, insurance, health literacy, and socioeconomic factors to overcome barriers to wellness.

Responsibilities

• Provide health management guidance and education to patients and families.
• Coordinate care by understanding each patient's unique needs and circumstances.
• Assist patients in overcoming barriers related to socioeconomic factors and health literacy.

Requirements

• Associate's Degree in Nursing with at least 2 years of nursing experience.
• Current BLS certification from AHA or ARC.
• Valid Registered Nursing License (state-specific or multi-state compact).

Preferred Qualifications

• Bachelor's Degree in Nursing.
• 1+ year in Case Management.
• National Case Management certification.
