Role Summary
The Care Coordinator is an experienced clinical professional responsible for providing guidance, education, and care coordination to a designated patient population. They assess patients' medical needs, insurance coverage, health literacy, and socioeconomic factors to support wellness and address barriers.
Key Responsibilities
- Manage and coordinate care for patients, ensuring effective health management.
- Educate patients and families to promote health and wellness.
- Identify and help overcome barriers related to socioeconomic and health factors.
Qualifications & Skills
- Associates Degree in Nursing with at least 2 years of nursing experience.
- Current BLS certification and a valid Registered Nursing License (Georgia or multi-state license).
- Possessing a Bachelor's Degree in Nursing is preferred, with over 1 year of Case Management experience.
- National certification in Case Management is also preferred.
