Care Coordinator RN - Part Time / Weekends

Role Summary

The Care Coordinator is an experienced clinical professional responsible for providing guidance, education, and care coordination to a designated patient population. They assess patients' medical needs, insurance coverage, health literacy, and socioeconomic factors to support wellness and address barriers.

Key Responsibilities

• Manage and coordinate care for patients, ensuring effective health management.
• Educate patients and families to promote health and wellness.
• Identify and help overcome barriers related to socioeconomic and health factors.

Qualifications & Skills

• Associates Degree in Nursing with at least 2 years of nursing experience.
• Current BLS certification and a valid Registered Nursing License (Georgia or multi-state license).
• Possessing a Bachelor's Degree in Nursing is preferred, with over 1 year of Case Management experience.
• National certification in Case Management is also preferred.
