Role Overview

The Care Coordinator is an experienced clinical professional responsible for guiding and educating patients and families, managing their health, and coordinating care for a designated patient population.

Key Responsibilities

• Provide personalized support by understanding patients' medical needs, insurance, health literacy, and socioeconomic factors to overcome barriers to wellness.
• Aid patients in navigating healthcare systems and promoting health management.

Qualifications & Skills

• Requires an Associates Degree in Nursing, 2+ years of nursing experience, current RN licensure, and BLS certification.
• PREFERRED: Bachelor's Degree in Nursing, at least 1 year of case management experience, and national certification in case management.
