Role Overview The Care Coordinator is a skilled clinical professional responsible for guiding and educating patients and families, managing their health, and coordinating care for a designated patient population. They assess patients' medical needs, insurance, health literacy, and socioeconomic factors to address barriers to wellness. Key Responsibilities Providing patient education and support to promote health and wellness.Coordinating care across services to ensure comprehensive patient management.Identifying and overcoming barriers related to socioeconomic factors and health literacy.Assisting patients in navigating healthcare systems and resources.Monitoring and managing individual patient cases to optimize outcomes. Qualifications & Skills Associates Degree in Nursing and minimum 2 years of nursing experience.Current BLS certification from AHA or ARC.Active Registered Nurse license (Georgia) or multi-state compact license.Preferred: Bachelor's Degree in Nursing, over 1 year of case management experience, and national certification in case management.
