Care Coordinator RN - Part Time / Weekends

Role Overview

The Care Coordinator is a skilled clinical professional responsible for guiding and educating patients and families, managing their health, and coordinating care for a designated patient population. They assess patients' medical needs, insurance, health literacy, and socioeconomic factors to address barriers to wellness.

Responsibilities

• Providing patient education and support to promote health and wellness.
• Coordinating care across services to ensure comprehensive patient management.
• Identifying and overcoming barriers related to socioeconomic factors and health literacy.
• Assisting patients in navigating healthcare systems and resources.
• Monitoring and managing individual patient cases to optimize outcomes.

Qualifications & Skills

• Associates Degree in Nursing and minimum 2 years of nursing experience.
• Current BLS certification from AHA or ARC.
• Active Registered Nurse license (Georgia) or multi-state compact license.
• Preferred: Bachelor's Degree in Nursing, over 1 year of case management experience, and national certification in case management.
