Role Overview

The Care Coordinator is an experienced clinical professional responsible for guiding and educating patients and families, managing their health, and coordinating care for a designated patient population.

Responsibilities

• Provide personalized support by understanding patients' medical needs, insurance coverage, health literacy, and socioeconomic factors.
• Help patients overcome barriers to wellness, ensuring comprehensive care management.

Qualifications & Skills

• Associates Degree in Nursing
• 2+ years of nursing experience
• Current Basic Life Support (BLS) certification
• Active Registered Nursing License (state-specific or multi-state compact)
• Preferred: Bachelor's Degree in Nursing
• Preferred: 1+ year of Case Management experience
• Preferred: national certification in Case Management
