About the role
The Care Coordinator is an experienced clinical professional responsible for providing guidance and education to patients and families.
Responsibilities
- Manage and coordinate care for a designated patient population.
- Assess medical needs, insurance coverage, health literacy, and socioeconomic factors to support patient wellness.
- Assist patients in overcoming barriers related to these factors.
Requirements
- Associate’s Degree in Nursing with at least 2 years of nursing experience.
- Current BLS certification.
- Valid Registered Nursing License (Georgia or multi-state).
Preferred Qualifications
- Bachelor’s Degree in Nursing.
- Over 1 year of Case Management experience.
- National Certification in Case Management.
