About the role
An experienced clinical professional responsible for guiding and educating patients and families to manage their health effectively.
Responsibilities
- Provide health management and care coordination for a designated patient population.
- Assess patients' medical needs, insurance coverage, health literacy, and socioeconomic factors.
- Assist patients in overcoming barriers to wellness related to these factors.
- Offer guidance and education to support patient health and wellness.
Qualifications and Skills
- Associates Degree in Nursing with at least 2 years of nursing experience.
- Current BLS certification from AHA or ARC.
- Valid Registered Nursing License (state-specific or multi-state compact).
- Preferred: Bachelor's Degree in Nursing, over 1 year of case management experience, and national certification in case management.
