RN, Care Coordinator - Part Time

About the role

The Nurse Care Coordinator at Saint Luke’s East is responsible for focusing on care transitions throughout the healthcare continuum. They serve as an integral member of a multi-disciplinary team, ensuring high-quality patient outcomes, monitoring resource usage, and facilitating interdisciplinary collaboration, communication, and coordination.

Responsibilities

• Develops and implements individualized discharge plans.
• Identifies additional services needed and makes appropriate referrals.
• Facilitates multi-disciplinary rounds.
• Ensures clinical readiness for discharge.
• Collaborates with multidisciplinary teams.
• Manages throughput and mitigates delays in care.
• Collaborates with attending physicians to determine appropriate length of stay and monitors resource utilization.
• Affirms patient needs and implements a timely discharge plan with patients, families, and caregivers.
• Maintains accurate documentation.

Requirements

• A valid MO RN license and 1 year of RN experience are required.
• A BSN degree is required within 3 years of starting the position.
