SUMMARY

The Service Coordinator is directly responsible for the service provision and housing assistance for the residents in the Affordable Housing Portfolio. This program serves seniors and persons with disabilities located at one of the affordable housing sites. Service modalities include Trauma Informed Care, Harm Reduction, and Motivational Interviewing. This position will work in conjunction with the Community Administrator at the housing site.

ESSENTIAL DUTIES AND RESPONSIBILITIES

- Ensures that the resident's voice is heard, and their ideas play a prominent role in programming. - Within program guidelines, facilitates and accepts referrals on residents from the Community Administrator, family members, service providers, and the residents themselves. - Assesses services needed by residents in the building through surveys, visitations, group meetings, etc. Complete an assessment and collect needed information. Responsible for identifying and serving the unique needs of the resident and utilizing screening and assessment. - Facilitates the delivery of appropriate services for the households as indicated by relevant service modalities through an equity lens. - Provides the residents information on specific services and/or program providers, giving several options whenever possible. Complies with all Fair Housing requirements. - Encourages the resident and/or family members to arrange for required services when possible, to maintain and encourage their independence. - Coordinates and monitors the services and activities of providers to residents to assure quality of care and cost effectiveness. Advocate for residents per their wishes. - Maintains up-to-date files on service providers in the community by installing and becoming knowledgeable with the United Way database and Benefits Check-up. - Maintains and submits accurate and updated records on all residents to include the services they receive. - Develops support groups and workshops in response to needs and facilitates / co-facilitates these groups on a regular basis. - Assists in relocating residents as needed and securing emergency housing. - Supports team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field. - Responsible for implementing the Service Coordinator program in accordance with HUD and VOA guidelines. - Attends orientations, trainings, education programs, staff meetings, community meetings, conferences and workshops as requested and applicable to meet the needs of the position. May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices. - Clearly documents all client interactions along with required eligibility and demographic information. - Performs duties in a professional manner by maintaining the confidentiality of all information and by participating effectively within and across teams. - Must complete annual CEU requirements. - Performs job responsibilities in accordance with the Social Work Code of Ethics. - Performs all other duties as assigned.

MINIMUM QUALIFICATIONS OF POSITION

- Bachelor’s degree in human services, social work, or a closely related field or related experience. - At minimum, one full year of working with senior citizens and/or people with disabilities. - Willing to obtain first aid, automated external defibrillator and cardiopulmonary resuscitation certifications and keep current. - Criminal and civil background checks within 30 days of hire. - Excellent interpersonal skills and good organizational skills. - Proficient in using Windows environment and working with databases.

PREFERRED QUALIFICATIONS OF POSITION

- Mental health experience preferred. - Bilingual in a language represented in the resident community (Mandarin Chinese, Vietnamese, or Spanish) is a plus.

KNOWLEDGE AND SKILLS

- Expert written, oral, and interpersonal communication skills. - High level of competency working with Microsoft Office suite and cloud-based applications. - Strong time-management and prioritization skills. - Experience working both independently and, in a team-oriented, collaborative environment. - Strong organization skills.
