About the role
The Service Coordinator at VOAC is responsible for the service provision and housing assistance for residents at Grand View in the Affordable Housing Portfolio. This role serves seniors and persons with disabilities and includes Trauma Informed Care, Harm Reduction, and Motivational Interviewing.
Responsibilities
- Conducts needs-based assessments and community outreach
- Provides case management services and supports housing retention
- Connects residents to income resources and/or public benefits
- Attends meetings and collaborates with care providers and multi-disciplinary teams
- Facilitates health, wellness, and educational programs tailored to resident needs
- Maintains an up-to-date resource guide on service providers in the community
- Develops and facilitates support groups and workshops
- Supports team members by serving as a resource for problem-solving and staying informed about current issues and theories
- Ensures compliance with Fair Housing requirements
- Documents all client interactions and maintains accurate records
- Performs duties in a professional manner, maintaining confidentiality and participating effectively within and across teams
Requirements
- Bachelor’s degree in human services, social work, or a closely related field or related experience
- One full year of working with senior citizens and/or people with disabilities
- Willingness to obtain certifications in first aid, automated external defibrillator, and cardiopulmonary resuscitation
- Criminal and civil background checks within 30 days of hire
- Excellent interpersonal and organizational skills
- Proficiency in using Windows environment and working with databases
- Experience working independently and in a team-oriented, collaborative environment
- Strong time-management and prioritization skills

Pay
$20.00-$22.00/hr (Based on experience)
Working Conditions and Physical Requirements
- Indoor office environment with prolonged periods of sitting
- Some local travel required for training and meetings
- Out-of-state travel once per year for national conferences

Position Type and Expected Hours of Work
Full-time, 40 hours per week. Monday-Friday, 8 AM – 5 PM
