What You’ll Be Doing
- Manage a caseload of senior tenants, focusing on housing retention, emotional well-being, and social integration.
- Provide monthly in-home assessments of tenants' social and emotional needs; evaluate functioning, monitor risk, and update case plans accordingly.
- Aid formerly unhoused tenants with community transitions; ensure all tenants maintain the subsidies and benefits required for permanent housing.
- Develop and implement comprehensive case plans, including individual and family counseling to resolve tenancy and personal issues.
- Screen for eligibility and assist tenants with applications for appropriate benefits and entitlements.
- Facilitate tenant participation in group meetings and recreational activities to build informal support networks with family, friends, and neighbors.
- Connect tenants to legal aid, primary care, and mental health services; educate residents on their rights, obligations of tenancy, and service availability.
- Consult with supervisors regarding high-risk tenants or those at risk of eviction; report on common problematic themes or patterns within the tenant community.
What You’ll Need
- Education: Bachelor’s degree (BSW preferred) in Sociology, Gerontology, Psychology, Human Services, or a related field.
- Experience: Minimum of one year of relevant experience is required.
- Technical Skills: Proficiency in Microsoft Office, Google Drive, and electronic case record systems.
- Communication: Excellent interpersonal and multitasking skills; Bilingual English/Spanish is preferred.
