Registered Nurse (RN) - Home Health Case Management

Responsibilities

• Manage a caseload of home health patients, supervise LPN visits, and coordinate with therapy teams.
• Conduct assessments and develop personalized care plans aligned with physician orders.
• Support patients with conditions such as CHF, COPD, diabetes, wounds, and medication management.

Qualifications

• Missouri or Compact RN license.
• Valid driver’s license.
• Strong organizational, clinical, and communication skills.

Schedule

Monday–Friday schedule with minimal on‑call duties.
