About the role
We are seeking a Registered Nurse (RN) Home Health Case Manager to provide comprehensive patient care in clients' homes. Coordinate services with healthcare providers, including physical, speech, and occupational therapists. Develop personalized care plans based on assessments and physician orders. Supervise LPN visits and manage a varied caseload involving routine visits, evaluations, and care starts. Educate patients on health conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management. Communicate effectively with physicians and insurance companies to ensure quality care delivery. Utilize AI-supported charting tools to enhance efficiency and work-life balance.
Responsibilities
- Managing caseloads and supervising nursing staff

Qualifications
- Current Missouri or Compact RN license
- Valid driver’s license with auto insurance
- Experience in home health or critical care preferred
- Strong physical assessment, diagnostic interpretation, and communication skills
