About the role
Registered Nurse (RN) Home Health Case Manager, responsible for delivering personalized patient care in patients' homes, coordinating healthcare services, and overseeing visiting nurses.
Key Duties
- Manage caseloads
- Conduct assessments
- Develop and implement care plans aligned with physician orders
- Provide patient education on conditions such as CHF, COPD, Diabetes, and Wound Care
- Collaborate with physicians and therapy teams including PT, OT, and Speech
- Supervise LPN visits
- Ensure effective communication with insurance providers
- Utilize AI-supported charting tools to enhance efficiency and work-life balance
Qualifications & Skills
- Kansas or Compact RN licensure
- Valid driver's license and vehicle with insurance
- Preferred experience in Home Health or Critical Care
- Strong physical assessment, diagnostic interpretation, and communication skills
