About the role
We are seeking a Registered Nurse (RN) Home Health Case Manager for a full-time role in Wichita, Kansas.
Responsibilities
- Manage a caseload of home health patients
- Supervise LPN visits
- Coordinate with therapy teams
- Conduct assessments and develop personalized care plans aligned with physician orders
- Support patients in managing conditions such as CHF, COPD, diabetes, wounds, and medication regimens
Qualifications
- Valid Kansas or Compact RN license
- Valid driver's license
- Strong organizational, clinical, and communication skills
