About the role
We are seeking a dedicated Registered Nurse (RN) Home Health Case Manager to provide compassionate, patient-centered care in clients' homes.
Responsibilities
- Manage a diverse caseload, including routine visits, evaluations, and care starts, ensuring personalized treatment plans.
- Coordinate with healthcare providers, including physical, speech, and occupational therapists, to deliver comprehensive care.
- Supervise LPN visits and oversee service delivery to maintain high-quality standards.
- Communicate effectively with physicians to obtain orders and develop diagnosis-driven care plans.
- Educate patients on health conditions such as CHF, COPD, Diabetes, wound care, and medication management.
- Utilize AI-powered charting tools to streamline documentation, supporting work-life balance.
Requirements
- Maintain valid Kansas or Compact RN licensure, along with a driver's license and vehicle with insurance.
Qualifications
- Preferred experience in home health or critical care settings.
- Key skills include strong assessment, diagnostic interpretation, communication, and teamwork.
