Registered Nurse (RN) - Home Health Case Management

About the role

We are seeking a dedicated Registered Nurse (RN) Home Health Case Manager to provide patient-centered care in North Kansas City, MO. This role emphasizes effective patient management, interdisciplinary collaboration, regulatory compliance, and fostering meaningful relationships to improve quality of care.

Responsibilities

• Managing a personal caseload with supervision of LPN visits, scheduling, and delivering a variety of care such as routine visits, evaluations, and care starts
• Educating patients on health conditions like CHF, COPD, Diabetes, Wound Care, and Medication Management
• Collaborating with physicians to obtain orders and develop diagnosis-driven plans of care
• Working closely with therapy teams including PT, OT, and Speech

Qualifications and Skills

• Active Missouri or Compact RN License and valid Missouri Driver's License
• Strong clinical assessment and diagnostic interpretation skills
• Organized, self-directed, compassionate communicator with patients, families, and physicians
• Home health experience preferred
