Responsibilities
- Manage caseloads
- Supervise LPN visits
- Conduct evaluations
- Initiate care
- Develop personalized care plans based on assessments and physician orders
- Collaborate with healthcare professionals in Physical Therapy, Speech Therapy, and Occupational Therapy
- Educate patients on health conditions such as CHF, COPD, Diabetes, Wound Care, and Medication Management
- Communicate effectively with physicians and insurance providers to coordinate services
- Utilize AI-supported charting tools to enhance efficiency and work-life balance
Qualifications
- Valid Kansas or Compact RN license
- A driver’s license, vehicle with insurance
- PREFERRED: Experience in Home Health or Critical Care
Skills
- Strong physical assessment, diagnostic interpretation, and communication skills
