What You’ll Do

Oversee the complete operation and performance of the delivery of dialysis treatment and care under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility within state and federal regulations.

Use professional nursing knowledge, critical thinking skills, and dialysis experience to serve as a resource to staff, physicians, and physician assistants.

Minimum Qualifications

  • Graduate of an accredited nursing program required; ADN, BSN, MSN preferred
  • Current state licensure as a Registered Nurse
  • CPR certification (AHA BLS)
  • 18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months

Required Skills And Competencies

  • Maintain on-call status on an intermittent or regularly scheduled basis
  • Ability to satisfactorily communicate in English including reading, writing, speaking, and understanding
  • Computer proficiency

Essential Duties And Responsibilities

  • Ensure dialysis unit operates in compliance with all ESRD, HCFA, federal, AAMI, OSHA, and company policies and procedures
  • Train and develop Skilled Nursing Facility staff while providing clinical oversight to non-licensed support team, delegating tasks as appropriate
  • Monitor staff compliance with CMS ESRD and state regulations and Rendevor Dialysis policies and procedures
  • Assure patients receive adequate medical attention during dialysis treatments and dialysis care while a patient of the Skilled Nursing Facility in the dialysis unit
  • Conduct comprehensive assessments that include the medical, behavioral, pharmaceutical, and social needs of the patient, identify gaps in care and barriers to good health
  • Create and implement a care plan that addresses identified needs, removes barriers to care, and improves the health of the patient
  • Coordinate care by serving as the advocate and resource for the patient, their family, and their provider(s)
  • Facilitate continuity of care, spanning settings such as the home, hospital, skilled nursing facility, and acute care facility
  • Manage patients during periods of transitions of care to minimize avoidable readmissions
  • Assess the patient's knowledge of their renal condition and provide education and self-management support
  • Ongoing reassessment and follow-up to improve patient outcomes
  • Review and maintain updated policies and procedures on a regular basis
  • Develop and monitor quality control measures to assure continuous quality of patient care
  • Facilitate Quality Assurance Meetings
  • Participate in the IDT and QAPI committee and attend meetings as needed and/or requested
  • Identify and report concerns, issues, and opportunities for improvement when identified
  • Participate in performance improvement projects within licensure, certification, and scope of practice
  • Participate in annual QAPI training and other continuing education, as assigned, and required
  • Provide suggestions for alternate strategies for indicators not improving
  • Assist in building and maintaining a facility/clinic culture of safety and quality
  • Report errors and near misses so that steps can be taken to prevent recurrence
  • Report any negative feedback, complaints, grievances from other staff members, patients, and families
  • Use evidence-based guidelines and proper infection control practices
  • Maintain open communication with Skilled Nursing Facility Administrator, Medical Director, and Rendevor Dialysis management and staff
  • Utilize resources to improve skills, knowledge, and practice to assure continuous quality of patient care
  • Promote a teamwork approach by offering information, advice, and assistance to all unit staff members in a positive, courteous, and cooperative manner
  • Check email at least weekly and respond appropriately and timely; use company-provided email for all work-related email communications
  • Maintain regular and predictable attendance and punctuality
  • Adhere to all company policies and procedures
  • Perform other duties as assigned

Supervision

This position will report directly to the Area Manager.

Work Environment

This job exists in the medical unit of a post-acute facility. This role may include exposure to chemicals and blood-borne pathogens. This role routinely involves the use of both medical and standard office equipment.

Physical Demands

  • Regularly required to talk or hear
  • Frequently required to stand; walk; use hands to handle or feel; and reach with hands and arms
  • Requires ability to push, pull or lift supplies and products up to 50 pounds
  • Specific visual abilities required by this job include close vision and ability to adjust focus
