What You’ll Do
Oversee the complete operation and performance of the delivery of dialysis treatment and care under the terms of the contract between Rendevor Dialysis and the Skilled Nursing Facility, adhering to state and federal regulations.
- Use professional nursing knowledge, critical thinking skills, and dialysis experience to serve as a resource to staff, physicians, and physician assistants.
Minimum Qualifications
- Graduate of an accredited nursing program required; ADN, BSN, MSN preferred.
- Current state licensure as a Registered Nurse.
- CPR certification (AHA BLS).
- At least 18 months of clinical nursing experience to patients on maintenance dialysis within the last 24 months.
Required Skills and Competencies
- Maintain on-call status on an intermittent or regularly scheduled basis.
- Ability to communicate effectively in English, including reading, writing, speaking, and understanding.
- Computer proficiency.
Essential Duties and Responsibilities
In conjunction with the Vice President, ensure the dialysis unit operates in compliance with all relevant regulations and policies.
- Train and develop Skilled Nursing Facility staff while providing clinical oversight.
- Monitor and ensure patients receive adequate medical attention during dialysis treatments and dialysis care.
- Conduct comprehensive assessments and create and implement care plans addressing medical, behavioral, pharmaceutical, and social needs.
- Coordinate care, advocate for patients, and facilitate continuity of care across various settings.
- Manage patients during transitions of care to minimize avoidable readmissions.
- Provide ongoing education and support for patients' self-management of their renal conditions.
- Review and update policies and procedures regularly.
- Implement and monitor quality control measures to ensure continuous quality of patient care.
- Participate in Quality Assurance Meetings, IDT, and QAPI committees.
- Report concerns, issues, and opportunities for improvement.
- Participate in performance improvement projects and annual QAPI training.
- Offer suggestions for alternative strategies and assist in building a safe and quality-focused facility culture.
- Communicate effectively with Skilled Nursing Facility Administrator, Medical Director, and Rendevor Dialysis management and staff.
- Use evidence-based guidelines and proper infection control practices.
- Ensure regular communication via email and adhere to company policies and procedures.
- Work regular shifts, ranging from five to fifteen hours per day, based on unit requirements.
Supervision
This position reports directly to the Area Manager.
Work Environment
The job exists in the medical unit of a post-acute facility. Exposure to chemicals and blood-borne pathogens may occur. Routine use of medical and standard office equipment is required.

- Push, pull, or lift supplies and products up to 50 pounds.
- Close vision and ability to adjust focus required.
