About the role

Provide high-quality home healthcare to pediatric patients with complex needs, including G-tube, Trach, and Vent management. Ensure clinical outcomes through compassionate care, documentation, and adherence to safety standards.

Responsibilities

• Delivering patient-centered nursing care in home settings
• Documenting care electronically using provided tablets
• Collaborating with a multidisciplinary team
• Supporting family education and patient comfort

Qualifications & Skills

• Active, unencumbered LPN/LVN license in the practicing state
• Current CPR certification (training available post-hire)
• TB test within last 12 months
• Reliable transportation
• Preferred: Six months of hands-on nursing experience
