About the role

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their given services. The Physical Therapist is an integral healthcare provider accountable to assist the patient to their highest level of function. The physical therapist will uphold the mission, vision, and values of Jefferson Health and works in conjunction with other health care professionals to coordinate treatment plans and care.

Responsibilities

• Provide therapy evaluations, treatment planning, treatment interventions and discharge planning for a full patient caseload in acute care, skilled facility, acute rehab, outpatient settings as appropriate.
• Plan, organize and deliver treatment programs that help clients/patients recover and rehabilitate.
• Utilize tools and techniques to administer safe therapeutic interventions and trainings, for patient and families.
• Record and document clients/patients' health progress information for evaluation, including all therapeutic interventions such as daily treatments, progress notes, evaluations and discharge summaries.
• Perform other duties as assigned.

Requirements

• Bachelor’s, Master’s, or Doctoral Degree from an accredited Physical Therapy Program.
• 0–2 years of clinical practice required; 1 year preferred.
• Valid PA state license in Physical Therapy.
• CPR certification required via AHA.
