About the role

Responsible for coordinating and delivering physical therapy services, including evaluating patients, developing individualized treatment plans, and implementing therapy using various strategies. Ensures effective communication with patients, families, and interdisciplinary team members to support patient progress.

Responsibilities

• Assess patients through data collection and observations.
• Create and review personalized treatment programs, instruct patients and caregivers, and adjust interventions based on ongoing evaluations.
• Provide direct therapy across multiple settings, ensuring care aligns with medical and rehabilitation plans.
• Document evaluations, treatment plans, and progress notes accurately.
• Communicate effectively with team members and participate in care conferences.
• Supervise and oversee physical therapy assistants, technicians, and students to ensure safe, efficient care.
• Maintain knowledge of equipment and uphold safety standards.

Requirements

• Active license as a Physical Therapist in the practice state.
• Strong communication skills, visual acuity, and ability to work collaboratively.
