About the role

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and evaluating patient progress to promote health and recovery.

Responsibilities

• Evaluate patients using various data sources.
• Develop and implement personalized treatment programs.
• Instruct patients, families, and caregivers on exercises and home programs.
• Provide direct therapy in diverse settings, aligning with medical and rehabilitation plans.
• Prepare documentation including evaluations, treatment plans, and progress notes.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Collaborate with team members and attend care conferences.
• Participate in professional development activities and maintain knowledge of equipment use.
• Supervise physical therapy assistants, techs, and students to ensure safe and efficient care.

Requirements

• Active license as a Physical Therapist in the practice state.
• Ability to perform essential functions, including effective communication, visual acuity, and hearing.
• Commitment to teamwork, confidentiality, and compliance with safety and regulatory standards.
