About the Role

Responsible for coordinating and delivering physical therapy services, including evaluating patients, developing individualized treatment plans, and instructing patients and families on exercises and home programs. Provides direct therapy in various settings, ensuring treatment aligns with patient capabilities and interdisciplinary care plans. Maintains knowledge of equipment use and promotes professional growth through ongoing education.

Responsibilities

• Evaluate patients and gather comprehensive data.
• Develop and implement individualized treatment programs.
• Provide direct therapy services and instruct patients and caregivers.
• Document evaluations, treatment plans, and progress notes.
• Collaborate with interdisciplinary team members and communicate patient progress.
• Supervise therapy assistants and students to ensure quality care.
• Maintain knowledge of equipment and safety standards.

Requirements

• Must hold an active physical therapy license in the practice state.
• Strong communication and visual acuity.
• Ability to work effectively in a team environment.
• Commitment to confidentiality, safety regulations, and organizational standards.

Qualifications

Preferred candidates are licensed physical therapists with experience in patient evaluation and treatment planning.
