About the role

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and evaluating patient progress within a multidisciplinary team.

Responsibilities

• Evaluate patients through various sources, including records and observations.
• Design and implement personalized treatment programs, instructing patients and caregivers.
• Use targeted activities to promote health, adapt interventions based on ongoing assessments.
• Provide direct therapy in diverse settings, aligning with medical and rehabilitation plans.
• Maintain thorough documentation such as evaluations, treatment plans, and progress notes.
• Communicate effectively with patients, families, and healthcare team members.
• Collaborate with interdisciplinary team members and participate in care conferences.
• Supervise and oversee physical therapy assistants, technicians, and students.
• Ensure proper use of equipment and participate in professional development activities.

Requirements

• Active state license as a Physical Therapist.
• Ability to perform essential functions, including effective communication and visual acuity.
