Position Overview

Responsible for coordinating and delivering physical therapy services, developing individualized treatment plans, and providing direct therapy to patients across various settings.

Key Responsibilities

• Evaluate patients through data collection like records, interviews, and observations.
• Develop and implement personalized treatment programs, review plans with patients, and instruct on home exercises.
• Use functional activities to promote health, modify interventions based on ongoing assessments, and coordinate with medical teams.
• Document evaluations, treatment plans, progress notes, and reports as required.
• Communicate effectively with patients, families, physicians, and interdisciplinary team members.
• Oversee and collaborate with physical therapy assistants, technicians, and students to ensure safe and effective patient care.
• Maintain knowledge of equipment use and participate in professional development activities.

Qualifications & Skills

• Active state licensure as a Physical Therapist.
• Ability to perform essential functions, including effective communication and visual acuity.
• Commitment to confidentiality, teamwork, and adherence to safety standards.
