About the role
Provide case management services to patients in their homes, coordinating care with other healthcare providers to ensure optimal patient outcomes.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Communicate with patients, families, and other healthcare professionals to coordinate care
- Monitor patient progress and adjust treatment plans as necessary
- Ensure compliance with regulatory standards and insurance requirements
- Document all interactions and treatments accurately and timely
Requirements
- Bachelor’s degree in Physical Therapy or related field
- Current license to practice as a Physical Therapist in California
- Experience in home health or similar setting preferred
- Strong communication and organizational skills
Qualifications
- Excellent interpersonal skills
- Proficient in Microsoft Office Suite
- Ability to work independently and as part of a team
