Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients receiving physical therapy services in their homes. Works closely with patients, families, and other healthcare providers to ensure effective treatment plans are implemented.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Communicate with patients, families, and healthcare providers to monitor progress and adjust treatment plans as necessary
• Provide education on home exercise programs and assist with patient compliance
• Document patient progress and communicate with supervising PTs
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor's degree in Physical Therapy from an accredited program
• Current California Physical Therapy license
• Experience in home health or related field preferred

Qualifications

• Strong interpersonal and communication skills
• Ability to work independently and manage multiple tasks simultaneously
• Proficient in Microsoft Office Suite

Pay
