Physical Therapist Case Manager: Home Health

About the role

Responsible for managing physical therapy cases in home health settings. Manage patient care plans, coordinate with other healthcare providers, and ensure compliance with regulatory standards.

Responsibilities

• Develop and implement individualized treatment plans for patients based on medical evaluations and assessments.
• Coordinate with other healthcare professionals to ensure comprehensive patient care.
• Monitor patient progress and adjust treatment plans as necessary.
• Communicate with patients and their families regarding treatment goals and outcomes.
• Ensure all services provided meet regulatory and accreditation standards.

Requirements

• Bachelor’s degree in Physical Therapy from an accredited program.
• Current California Physical Therapy license.
• At least 2 years of relevant work experience in home health or related field.
• Excellent communication and interpersonal skills.
• Proficiency in Microsoft Office Suite.
