Physical Therapist Case Manager: Home Health

About the role

The Physical Therapists (PT) Case Managers are responsible for managing the care of patients in their homes, ensuring they receive appropriate physical therapy treatments and support.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Communicate with patients, families, and other healthcare providers to coordinate care
• Monitor patient progress and adjust treatment plans as necessary
• Ensure compliance with insurance requirements and regulatory standards
• Document patient interactions and treatment sessions accurately

Requirements

• Bachelor’s degree in Physical Therapy or related field
• Current license to practice as a Physical Therapist in California
• Experience in home health or similar setting preferred
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively

Qualifications

• Strong clinical skills
• Knowledge of home health regulations and insurance requirements
• Proficiency in Microsoft Office Suite
