Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients in home health settings, ensuring they receive appropriate physical therapy treatments and follow-up care.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Coordinate with other healthcare providers to ensure comprehensive care
• Monitor patient progress and adjust treatment plans as necessary
• Communicate with patients and families about their care plan and progress
• Document all interactions and treatment sessions accurately

Requirements

• Bachelor’s degree in Physical Therapy
• Current license to practice as a Physical Therapist in California
• At least 2 years of experience in home health or related field
• Excellent communication and interpersonal skills

Qualifications

• Strong organizational and time management skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Skills

• Knowledge of home health care protocols and standards
• Experience with electronic medical records (EMR)
• Strong problem-solving and critical thinking abilities
