Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients receiving physical therapy services in home health settings. Works closely with patients, families, and other healthcare providers to develop and implement individualized treatment plans.

Responsibilities

• Assess patient needs and develop comprehensive treatment plans
• Coordinate with healthcare providers to ensure continuity of care
• Monitor patient progress and adjust treatment plans as necessary
• Communicate effectively with patients, families, and other healthcare providers
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor's degree in Physical Therapy or related field
• Current state licensure as a Physical Therapist
• Experience in home health or similar setting preferred
• Excellent communication and interpersonal skills

Qualifications

• Strong organizational and time management skills
• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite

Skills

• Knowledge of home health care practices and regulations
• Effective problem-solving and decision-making abilities
• Strong computer skills, including proficiency in electronic health records (EHR)
