Physical Therapist Case Manager: Home Health

Responsibilities

• Evaluate and treat patients in their homes, developing and implementing care plans.
• Supervise Physical Therapy Assistants (PTAs) as needed.
• Communicate with patients' physicians to ensure quality care.
• Report patients' progress to medical providers.

Requirements

• Current unrestricted CA Physical Therapist License.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Knowledge of local community resources.
• Experience with high acuity patients.
• Experience working with an interdisciplinary team.
• Case Management fundamentals.

Pay

Physical Therapist, Level 1: $120,000 - $135,000 annually, plus a $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.
