Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients receiving physical therapy services in their homes. Works closely with patients, families, and other healthcare professionals to develop and implement individualized treatment plans.

Responsibilities

• Assess patient needs and develop comprehensive treatment plans
• Provide direct patient care including therapeutic exercises, modalities, and education
• Coordinate with other healthcare providers to ensure continuity of care
• Document patient progress and communicate with referring physicians and other team members
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor’s degree in Physical Therapy from an accredited program
• Current California Physical Therapy license
• Experience in home health or related field preferred
• Excellent communication and interpersonal skills
• Ability to work independently and manage multiple tasks

Pay
