Physical Therapist Case Manager: Home Health

About the role

The Physical Therapist Case Manager will oversee the care coordination process for patients receiving home health services. They will work closely with patients, families, and other healthcare providers to ensure that patients receive the highest quality of care in their homes.

Responsibilities

• Oversee the care coordination process for patients receiving home health services
• Work closely with patients, families, and other healthcare providers to ensure high-quality care in the home setting
• Develop and implement care plans based on patient needs and medical guidelines
• Communicate effectively with patients, families, and interdisciplinary team members to ensure continuity of care
• Monitor patient progress and make adjustments to care plans as necessary
• Ensure compliance with regulatory requirements and insurance company guidelines
• Document all interactions and care activities accurately and timely

Requirements

• Bachelor's degree in Physical Therapy or related field
• Current California Physical Therapy license
• Minimum 2 years of experience as a Physical Therapist
• Experience in home health care or similar setting preferred
• Excellent communication and interpersonal skills
• Strong organizational and time management skills
• Ability to work independently and as part of a team

Qualifications

• Knowledge of home health care regulations and standards
• Proficiency in computer software, including electronic health records systems
• Ability to travel to various locations within the service area as needed

Pay

$50 - $60 per hour, depending on experience and qualifications
