About the role
Responsible for managing the care of patients in their homes, ensuring they receive appropriate physical therapy treatments and follow-up care.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Provide direct physical therapy services to patients in their home environment
- Coordinate with other healthcare providers to ensure comprehensive patient care
- Document progress and communicate with patients, families, and referring physicians
- Adhere to regulatory standards and insurance requirements
Requirements
- Bachelor’s degree in Physical Therapy from an accredited program
- Current state licensure as a Physical Therapist
- Experience in home health or related field preferred
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
Qualifications
- Strong clinical skills
- Knowledge of home health regulations and insurance processes
- Proficiency in Microsoft Office Suite
