The Physical Therapist Case Manager will oversee the care coordination process for patients receiving home health services. This includes managing patient care plans, ensuring compliance with regulatory standards, and collaborating with other healthcare providers.

Responsibilities

• Oversee the care coordination process for patients receiving home health services
• Manage patient care plans to ensure they are compliant with regulatory standards
• Collaborate with other healthcare providers to coordinate patient care
• Assess patient needs and develop appropriate treatment plans
• Communicate with patients, families, and other healthcare professionals to provide updates on patient progress
• Ensure all documentation is accurate and up-to-date

Requirements

• Bachelor’s degree in Physical Therapy or related field
• Current state licensure as a Physical Therapist
• At least 2 years of experience in home health or related field
• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
