Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients receiving physical therapy services in their homes. Works closely with patients, families, and other healthcare providers to ensure patient safety and optimal recovery.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Provide direct patient care, including exercises, modalities, and education
• Coordinate with other healthcare professionals to ensure comprehensive care
• Document patient progress and communicate with referring physicians
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor’s degree in Physical Therapy
• Current California Physical Therapy license
• At least 2 years of clinical experience in home health or related field
• Excellent communication and interpersonal skills
• Ability to work independently and manage time effectively

Qualifications

• Strong knowledge of physical therapy principles and techniques
• Proficiency in using electronic medical records systems
• Experience with home health equipment and supplies
