Physical Therapist Case Manager: Home Health

About the role

Responsible for managing the care of patients receiving physical therapy services in their homes. Works closely with patients, families, and other healthcare providers to develop and implement individualized treatment plans.

Responsibilities

• Assess patient needs and develop comprehensive treatment plans
• Communicate with patients, families, and other healthcare providers to coordinate care
• Provide direct patient care including exercises, modalities, and education
• Document patient progress and communicate with referring physicians
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor’s degree in Physical Therapy from an accredited program
• Current California Physical Therapy license
• At least 2 years of clinical experience in home health or related field
• Strong interpersonal and communication skills
• Proficiency in Microsoft Office Suite
