Responsible for managing the care of patients in their homes, ensuring they receive appropriate physical therapy treatments and monitoring their progress.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Provide direct physical therapy services to patients in their homes
• Monitor patient progress and adjust treatment plans as necessary
• Communicate with referring physicians and other healthcare providers
• Ensure compliance with regulatory standards and insurance requirements

Requirements

• Bachelor's degree in Physical Therapy
• Current state licensure as a Physical Therapist
• At least 2 years of clinical experience in home health or related field
• Excellent communication and interpersonal skills

Qualifications

• Ability to work independently and as part of a team
• Proficiency in Microsoft Office Suite
