Responsible for managing physical therapy cases in home health settings, ensuring patients receive appropriate care and treatment plans.
Responsibilities
- Assess patient needs and develop individualized treatment plans
- Communicate with patients, families, and other healthcare providers to coordinate care
- Monitor patient progress and adjust treatment plans as necessary
- Document patient interactions and treatment sessions accurately
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Physical Therapy
- Current license to practice as a Physical Therapist in California
- At least 2 years of experience in home health or related field
- Strong communication and organizational skills
Qualifications
- Proficient in Microsoft Office Suite
- Familiarity with electronic health record systems
- Ability to travel within the Bay Area
