About the role

Responsible for managing the care of patients receiving physical therapy services in their homes. Manage patient care plans, coordinate with other healthcare providers, and ensure compliance with regulatory standards.

Responsibilities

• Assess patient needs and develop individualized treatment plans
• Communicate with patients, families, and other healthcare professionals to monitor progress and adjust treatment plans as necessary
• Ensure all treatments are performed safely and effectively
• Document patient interactions and treatment sessions accurately
• Collaborate with interdisciplinary teams to provide comprehensive care
• Adhere to regulatory requirements and maintain compliance

Requirements

• Bachelor's degree in Physical Therapy
• Current license to practice as a Physical Therapist in California
• Experience in home health care setting preferred
• Excellent communication and interpersonal skills
• Proficiency in computer systems for documentation and scheduling
