About the role
This role provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams and delivering advanced nursing care in various settings.
Responsibilities
- Evaluate patient responses and develop care plans.
- Conduct post-discharge and assessment visits.
- Coordinate with patients, families, and other healthcare providers.
- Ensure care aligns with regulatory standards.
- Manage team performance and support continuous improvement initiatives.
- Perform procedures within scope of practice.
- Oversee documentation quality.
- Travel to patient locations as needed.
- Act as preceptors for staff.
- Participate in quality committees.
- Support staff education and training.
Requirements
- Current licensure and registration as a Registered Nurse and Nurse Practitioner in New York State.
- Specialty certification (adult, family, gerontology, or psychiatric for Psychiatric Nurse Practitioners).
- Master’s Degree in Nursing from an accredited program; ANCC or AANP certification required.
- Psychiatric Nurse Practitioners: psychiatric-mental health Nurse Practitioner certification; PhD preferred.
- Minimum two years of full-scope nurse practitioner experience, with home care or managerial background preferred.
- Psychiatric Nurse Practitioners: two years of psychiatric practice required.
- Knowledge of quality measures, coding, and reimbursement practices.
- Bilingual skills may be required based on operational needs.
