Role Overview

Provides clinical leadership to enhance compliance with quality and care standards, managing service delivery of inter-professional teams for individual or population-based care. Acts as a clinical resource, offering guidance to achieve optimal outcomes while delivering advanced nursing care in accordance with state, federal, and national standards. Manages comprehensive, advanced nursing practice for targeted populations, including assessments, treatment plans, and interventions such as post-discharge visits, palliative care, and urgent care escalations.

Responsibilities

  • Collaborates with patients, families, and healthcare team members to develop and implement care plans, ensuring continuity and quality of care across settings.
  • Ensures compliance with patient-centered medical home standards, quality measures, and documentation requirements.
  • Participates in team activities, quality improvement, peer reviews, and may serve as a preceptor or committee member.
  • Performs procedures as authorized, provides telehealth or home-based care, and manages clinical documentation and billing.
  • May include psychiatric assessments and treatments for eligible Nurse Practitioners, involving psychosocial evaluations, medication management, and crisis intervention.

Qualifications

  • Licensed and registered as a Nurse Practitioner in New York State, with certification by ANCC or equivalent.
  • Requires master's degree in nursing, with specialty in adult, family, gerontology, or psychiatric-mental health (for psychiatric NPs).
  • Minimum two years of clinical experience.
  • Proficiency in coding, quality measures, and bilingual skills as needed.
  • Psychiatric NPs must have relevant mental health certification and experience.
