About the role
Provides clinical leadership to enhance quality and cost compliance, managing inter-professional teams for individual cases or populations. Acts as a key resource for clinical and operational guidance, delivering advanced nursing care within regulatory standards. Works under the supervision of the Clinical Director for the Nurse Practitioner Program.
Responsibilities
- Provides full scope of advanced nursing practice, including assessments, care planning, interventions, and discharge planning.
- Collaborates with patients, families, and care teams to ensure comprehensive, patient-centered care.
- Manages quality metrics, documentation, billing, and compliance standards.
- Performs procedures per collaborative agreements and provides care across various settings, including virtual and home visits.
- Participates in interdisciplinary meetings, quality improvement, and peer reviews.
- May serve as a preceptor and contribute to clinical model development.
- For psychiatric roles, conducts psychosocial assessments, provides mental health interventions, and collaborates with psychiatrists.
Qualifications and Skills
- Requires licensure as a Registered Nurse and Nurse Practitioner certification in New York, with specialty-specific certifications.
- Master’s degree in Nursing and ANCC or AANP certification are mandatory; psychiatric practitioners need additional mental health certification.
- Minimum two years of practice experience preferred, with knowledge of quality measures, coding, and care documentation.
- Bilingual skills may be required based on operational needs.
