About the role

Provides clinical leadership, manages comprehensive advanced nursing care, and supervises inter-professional teams to ensure quality, cost-efficiency, and compliance with standards. Acts as the clinical authority in care planning, leading interdisciplinary rounds, and delivering patient-centered services across various settings, including virtual and home visits.

Responsibilities

  • Conducts assessments
  • Develops treatment plans
  • Manages post-discharge and chronic care visits
  • Performs urgent interventions
  • Collaborates with patients, families, and care teams to optimize outcomes
  • Ensures documentation accuracy
  • Participates in quality improvement initiatives
  • Supports team performance
  • Performs procedures within scope
  • Provides patient education
  • Ensures 24/7 coverage
  • May act as a preceptor and participate in special projects or committees

Qualifications

  • Current RN license and nurse practitioner certification in New York, with specialty in adult, family, gerontology, or psychiatric health
  • Graduate-level education
  • ANCC or equivalent certification
  • Experience utilizing full scope of practice
  • Psychiatric NPs must hold PMHNP-BC certification and have relevant mental health experience
  • Bilingual skills (preferred)
  • Knowledge of quality measures (HEDIS, QARR), coding, and documentation standards (preferred)
