About the role
Provides clinical leadership, manages comprehensive advanced nursing care, and supervises inter-professional teams to ensure quality, cost-efficiency, and compliance with standards. Acts as the clinical authority in care planning, leading interdisciplinary rounds, and delivering patient-centered services across various settings, including virtual and home visits.
Responsibilities
- Conducts assessments
- Develops treatment plans
- Manages post-discharge and chronic care visits
- Performs urgent interventions
- Collaborates with patients, families, and care teams to optimize outcomes
- Ensures documentation accuracy
- Participates in quality improvement initiatives
- Supports team performance
- Performs procedures within scope
- Provides patient education
- Ensures 24/7 coverage
- May act as a preceptor and participate in special projects or committees
Qualifications
- Current RN license and nurse practitioner certification in New York, with specialty in adult, family, gerontology, or psychiatric health
- Graduate-level education
- ANCC or equivalent certification
- Experience utilizing full scope of practice
- Psychiatric NPs must hold PMHNP-BC certification and have relevant mental health experience
- Bilingual skills (preferred)
- Knowledge of quality measures (HEDIS, QARR), coding, and documentation standards (preferred)
