Per Diem Advanced Practice Clinician

Role Overview

Provides clinical leadership to enhance quality care, manage service delivery, and ensure compliance with regulatory standards. Acts as a key resource for interdisciplinary teams, offering advanced nursing clinical care in various settings, including virtual, home, and facility-based environments.

Responsibilities

• Manages full scope of advanced nursing practice for targeted populations, evaluating responses to therapy and revising care plans to achieve optimal outcomes.
• Serves as clinical authority, leading rounds, interdisciplinary meetings, and providing post-discharge, assessment, palliative, and change-in-condition visits.
• Collaborates with patients, families, and healthcare providers for assessment and care planning, ensuring continuity across settings.
• Ensures care aligns with patient-centered standards, manages team efforts, and contributes to quality improvement initiatives.
• Participates in on-call coverage, patient education, and team performance assessments, providing remedial support as needed.
• Performs procedures within collaborative agreements, manages documentation, billing, and quality measures, and may serve as a preceptor or committee member.
• Supports mental health care for psychiatric nurse practitioners, including psychosocial assessment, medication management, and crisis intervention.

Qualifications

• Valid RN and Nurse Practitioner licenses in New York State with current registration.
• Certification by ANCC or equivalent in adult, family, or gerontology specialties; PMHNP-BC for psychiatric roles.
• Master's degree in Nursing; psychiatric nurse practitioners require a psychiatric-mental health degree.
• Minimum two years of experience as a nurse practitioner, with home care or managerial background preferred.
• Knowledge of quality measures, coding, and reimbursement; bilingual skills may be required.
