About the Role

Provides clinical leadership to enhance compliance with quality and care standards, managing inter-professional teams and delivering advanced nursing care. Acts as a clinical authority, leading patient assessments, care planning, and interventions across various settings, including virtual and home visits. Ensures care aligns with regulations, standards, and quality measures such as HEDIS and QARR. Collaborates with patients, families, and healthcare team members to develop and revise treatment plans, manage chronic conditions, and support patient education. Performs procedures under collaborative agreements, oversees documentation, and participates in quality improvement initiatives. May serve as a preceptor, contribute to clinical model development, and support staff education.

For Psychiatric Nurse Practitioners, conducts psychosocial assessments, provides mental health interventions, and collaborates with psychiatrists.

Responsibilities

  • Lead patient assessments, care planning, and interventions in various settings (virtual, home visits, etc.).
  • Ensure compliance with healthcare regulations, standards, and quality measures (e.g., HEDIS, QARR).
  • Collaborate with patients, families, and healthcare teams to develop and revise treatment plans.
  • Manage chronic conditions and support patient education.
  • Perform clinical procedures under collaborative agreements.
  • Oversee documentation and participate in quality improvement initiatives.
  • Serve as a preceptor, contribute to clinical model development, and support staff education.
  • For Psychiatric Nurse Practitioners: conduct psychosocial assessments and provide mental health interventions in collaboration with psychiatrists.

Qualifications

  • Valid RN and Nurse Practitioner licenses in New York State.
  • Certification by ANCC or equivalent.
  • Master’s Degree in Nursing; psychiatric practitioners require specific psychiatric-mental health certification.
  • Minimum two years of clinical or managerial experience as a nurse practitioner, with knowledge of coding and quality measures.
  • For psychiatric roles: at least two years of psychiatric NP experience.
  • Bilingual skills may be required.
