About the role
Provides clinical leadership to enhance quality and cost compliance, managing inter-professional teams delivering patient-centered care. Acts as a clinical resource, offering advanced nursing care within regulatory standards, under supervision of the Clinical Director.
Responsibilities
- Manage full scope of advanced nursing practice for targeted populations, including assessments, therapy evaluations, and care plan revisions.
- Lead interdisciplinary team rounds, coordinate comprehensive care such as post-discharge visits, assessments, palliative care, and urgent interventions.
- Collaborate with patients, families, and providers to develop and implement individualized care plans, ensuring quality outcomes in compliance with healthcare standards.
- Oversee clinical services across various settings—virtually, home visits, or facilities—perform procedures as authorized, and ensure accurate documentation and billing.
- Participate in quality improvement initiatives, audit work performance, and support education and training of staff and students.
- For psychiatric roles: conduct psychosocial assessments, develop treatment plans, provide mental health interventions, and collaborate with psychiatrists.
Qualifications
- Licensed Registered Nurse and Nurse Practitioner in New York State, with current certification (ANCC or equivalent).
- Master’s Degree in Nursing from an accredited program; psychiatric NPs require additional psychiatric-mental health certification and experience.
- Minimum two years of relevant clinical experience; home care or managerial experience preferred.
- Knowledge of quality measures, coding, and billing; bilingual skills may be required.
